PROCESS TO BECOME LICENSED

1. Call the Bangor DHHS at (207) 561-4100 and tell them you are interested in becoming a foster parent and they will set you up to attend the next monthly informational meeting.

2. At the end of the of the informational meeting, you will receive all of the required paperwork.

3. Fill out your application and self study, find three references who will fill out the forms in a timely fashion. Attested birth, marriage and divorce certificates. Latest IRS info. Medical form from your physician.

4. Attend your 24-hour pre-service training. DHHS provides this or CHOICES may if we have eight people who need it. You may not have children placed in your home until this is completed.

5. Cooperate with the home study process. This takes at least 10 hours of interviews over several weeks time and then the write up of the home study may take several more weeks.

6. You will need to have a water test if you are not on city water and a fire inspection to become licensed.

7. Be patient with the process as it can take longer depending on how many homes need home studies done at any one time.
